- DECLARATION OF A WIDOW FOR PENSION
State of Missouri
County of Camden
On this 13th day of February, A. D. one thousand nine hundred and One '1901'
Personally appeared before me, a Notary Public within and for the county and
State aforesaid, Martha E. Jacobs, aged 54 years, a resident near Stoutland, County of Camden, State of Missouri, who, being duly sworn according to law, makes the following declaration in order to obtain pension under the provisions of the act of Congress approved June 27, 1890, as amended by the act of May 9 1900
That she is the widow of Charles Jacobs Dec., who was 1st Sergeant under the name of Charles Jacobs on the 20th day of December, 1861 as a 1st Sergeant in Co. A, 11th Reg Ill Vols. And honorably discharged Sept 30th, 1865, having served ninety days or more during the late war of the rebellion. That the soldier was in the military or naval service of the United States except as stated above.

That she was married under the name of Martha E. Wallace to said soldier at Knoxville Ills on the 1st day of January, 1866, by Lewis Massey a J. P.: that there was no legal barrier to the marriage; that she had not been previously married;
That the said soldier died January 10th, 1901 near Stoutland Mo.; that she was not divorced from him; that she has not remarried since his death; and that she is without other means of support than her daily labor and actual net income not exceeding two hundred fifty dollars per year.
That the said soldier left the following-named children who are now living and under sixteen years of age, to wit:
Joseph W. Jacobs born July 17th, 1890, near Stoutland Mo.
That she has not heretofore applied for pension
That she hereby appoints Chas Jacobs War Pen Certif no 524.975 he true and lawful attorney, to prosecute this claim.
That her post-office address is Martha E. Jacobs, Stoutland
County of Camden, State of Missouri

Also personally appeared Georgina Ricord, residing at Stoutland Mo. And Clara Wilson, residing at ?(might be Hillhouse) Mo., persons whom I certify to be respectable and entitled to credit, and who, being by me duly sworn, say they were present and saw Martha E. Jacobs the claimant, sign her name to the foregoing declaration; that they have every reason to believe, from the appearance of said claimant and their acquaintance with her of 12 years and 5 years respectively, that she is the identical person she represents herself to be; that she is without other means of support than her daily labor and an actual net income of not exceeding two hundred and fifty dollars per year; and that they have no interest in the prosecution of this claim.
Sworn to and subscribed before me this 13th day of February, A. D. 1901
and I hereby certify that the contents of the above declaration, etc., were fully made known and explained to the applicant and witnesses before swearing, including the words (blank), erased, and the words (blank), added; and that I have no interest, direct of indirect, in the prosecution of this claim.
